INSURANCE PARTICIPATION

Urology Group of New Jersey physicians participate in most major insurance plans including:

Medicare
Horizon Blue Cross
Cigna
Amerihealth
Healthnet
Oxford
United

If you have another insurance plan, please contact your physician’s office directly to inquire about participation.

Insurance Coverage and Billing

UGNJ will submit claims on your behalf for participating insurance companies.  Co-pays are expected to be paid at the time of service and we will balance bill you for any additional cost-sharing or deductibles per your insurance benefit plan.  It is important that you review your benefit plan so you are informed of your out-of-pocket responsibility.  Nowadays, many benefit plans have a cost-sharing component.

For health plans that your physician does not participate, payment is due at time of service.  We will provide you a claim form upon request to submit to your insurance carrier for reimbursement.  If you are having a surgical procedure, a deposit will be required prior to scheduling your procedure—payment in full is due prior to the surgery date.  Payment arrangements can be accommodated upon request.
